letter-spacing: 0.034em;">指标及方案 福州一中等26所公办普高学校（或校区）特色创新班自主招生计划表见表一，各校（校区）具体招生方案将于5月23日前发布在福州阳光中招网。网址：https://jyj.fuzhou.gov.cn/zwgk/cswz/fzygzz/） 报名及审核 1.推荐形式：符合条件的各初中学校均有推荐资格和推荐名额，考生根据各招收学校的条件要求，可选择通过学校推荐、个人自荐、校长实名推荐等形式到福州阳光中招网进行网上报名。 2.网上报名：5月30日8:30至6月3日17:00，学生在监护人监护下，登录福州阳光中招网，在“应用系统链接—中招管理系统”中，选择“中考考生登录”，登录后选择“自主招生报名”菜单，根据系统提示选择要报名的学校及招生类型（系统限制每人只能报一所学校）。学生登录账号：中考报名时用的账号，即学籍辅号。 3.报名材料递交：根据普高学校的自主招生方案要求，学生或监护人于6月10日—11日向报考的自主招生学校递交报名材料或初中校长实名推荐材料。 4.资格审核查询：6月13日前，招生学校通过中招管理系统提交报名学生的资格审查结果。招生学校账号：yx+5位招生学校代码（如：福州三中账号yx60103）。6月16日上午10时后，考生用本人账号登录中招管理系统查询资格审核通过情况。 5.准考证领取：准考证由自主招生普高学校编排，考生根据普高学校自主招生方案的时间节点领取或打印准考证。 考试及录取 1.全省统一中考时间：6月19日—21日，所有考生均需参加中考。 2.学校综合测试：6月22日—23日，测试形式包括笔试、面试等，面试分数需当场告知考生。具体测试安排以招生学校方案为准。 3.报送自主招生成绩：6月24日上午11时前，招生学校登录中招管理系统提交所有参加综合测试考生的笔面试分数及普高特色创新班预录取名单。 4.自主招生预录取时间：经市中招办核查学生的学籍和户籍信息后，于6月25日上午10点后在福州阳光中招网及学校官网公布预录取学生名单，录取办法以招生学校方案为准。自主招生学校如同步在校园网公布预录取学生名单，需对学生姓名、学籍辅号信息作脱敏处理，避免学生个人信息泄露。同时，考生也可登录中招管理系统查询本人的预录取信息。考生被普高特色创新班预录取后，将不再填报普高（含综合高中班）志愿，但可填报中本贯通、五年专或中职学校志愿。 5.正式录取时间和要求：普高创新班自主招生正式录取考生需符合以下条件：（1）综合素质等级评定良以上(含良)，（2）考查科目全部合格，（3）道德与法治、物理、化学、历史成绩均达到C级（含C）以上，（4）中考成绩在普高（含综合高中班）最低投档控制线之上，（5）学校自主招生方案中规定的其他要求。中考成绩公布后，符合上述条件的考生确定为自主招生正式录取学生。录取通知书由自主招生学校于7月15日后到市中招办录取点领取，自主招生学校可附自行设计的录取通知书。未被自主招生正式录取的学生，其填报的中本贯通、五年专或中职学校志愿将在中招投档时生效。 <span leaf="" style="color: rgba(0, 0, 0, 0.9); letter-spacing: 0.034em;">工作要求 1.加强组织领导。各有关学校要切实加强组织领导，落实专门机构，配备专门人员，做好自主招生各个环节的工作。学校纪检部门要充分发挥监督作用，强化监管和督查。 2.认真做好推荐审核工作。各初中学校的主要领导，要本着诚信、认真的态度择优推荐优秀学生参加自主招生工作，及时进行资格审核并提供相关证明。 3.严格责任追究。各有关学校要坚持公开、公平、公正的原则，凡在招生、考试、考核评价中弄虚作假、营私舞弊者，根据教育部关于印发《中小学教师违反职业道德行为处理办法（2018年修订）》的通知，视情节轻重给予相应处理。 4.落实考评制度。通过自主招生录取的学生同等列入“2025年初中办学综合效益考评方案”进行考评，市教育局将在三年报考率、中考全科及格率、中考优秀率、中考平均分等方面确定适当参数，酌情将各初中学校通过自主招生录取的学生情况统计在内，确保学校不因推荐学生参加自主招生而影响质量考评。                         <span leaf="" style="color: rgba(0, 0, 0, 0.9);
